Chapter 4. Certification
IC 25-14.5-4-1
Qualifications
Sec. 1. To qualify for a certificate under this article, an individualmust do the following:
(1) Satisfy the requirements of IC 25-14.5-3.
(2) Satisfactorily complete an application for certification,furnished by the board, in accordance with the rules adopted bythe board. The application must be verified by the applicant andfiled at least thirty (30) days before the administration of theexamination.
(3) Pay the application, examination, and certification feesestablished by the board.
(4) Except to the extent that section 4 of this chapter applies,successfully pass the qualifying examination adopted by theboard as described in IC 25-14.5-5.

Issuance of certificate
Sec. 2. Except as provided in section 3 of this chapter, the boardshall issue a certificate to an individual who:
(1) meets the conditions set forth in section 1 of this chapter;and
(2) is otherwise qualified for certification under this article.

Refusal to issue certificate
Sec. 3. The board may refuse to issue a certificate to an applicantfor certification under section 2 of this chapter if:
(1) the applicant has been disciplined by an administrativeagency in another state or jurisdiction, and the board determinesthat the violation for which the applicant was disciplined has adirect bearing on the applicant's ability to practice competentlyin Indiana; or
(2) the applicant has been convicted of:
(A) an act that would constitute a ground for disciplinarysanction under IC 25-1-9; or
(B) a crime that has a direct bearing on the applicant's abilityto practice competently.

Reciprocity; failure of examination; application fee
Sec. 4. (a) Except as provided in subsection (b), the board mayissue a certificate to an applicant for certification if the applicantpresents evidence that the applicant has been issued a certificate in
a state that has requirements for certification that are, by the board'sdetermination, equivalent to the requirements for certification inIndiana. An applicant applying for a certificate under this subsectionis not required to take an examination given by the board underIC 25-14.5-5.
(b) The board may refuse to issue a certificate under subsection(a) if the applicant has failed an examination given by the boardunder IC 25-14.5-5.
(c) The fee an applicant for certification must pay for a certificateissued under subsection (a) shall be set by the board underIC 25-14.5-2-5.

Effectiveness
Sec. 5. All certificates shall be effective when issued by the board.

Display of certificate
Sec. 6. (a) A certified dietitian must display the certificate in aconspicuous part of the office in which the certified dietitianpractices nutrition therapy services.
(b) Whenever practicing the profession of dietetics outside of oraway from the office or place of business, the certified dietitian shallmake available to each patient the certified dietitian's name, officeaddress, and the number of the certificate.
