    Chapter 31. Registration Forms; Content

Duties of commission
    Sec. 1. (a) The commission shall prescribe the forms required orpermitted under NVRA or this article.
    (b) The election division shall make the forms available on thewebsite maintained by the election division. A form must be madeavailable so that an individual can download the form forcompletion.

Forms prescribed by commission
    Sec. 2. Except as provided under section 3 of this chapter, acircuit court clerk or board of registration shall use the formsprescribed by the commission under section 1 of this chapter.

Modifications
    Sec. 3. A circuit court clerk or board of registration may makeminor modifications to registration forms to use the form in thecounty registration records.

Forms used to apply for registration
    Sec. 4. (a) A form used to apply for registration at a licensebranch must comply with 42 U.S.C. 1973gg-3(c)(2) and 42 U.S.C.1973gg-3(d).
    (b) A form used to apply for registration at:
        (1) a public assistance agency designated under IC 3-7-15;
        (2) an agency serving persons with disabilities designated underIC 3-7-16;
        (3) an additional office designated under IC 3-7-18 orIC 3-7-19; or
        (4) an office of the department of employment and trainingservices designated under IC 3-7-20.5;
must comply with 42 U.S.C. 1973gg-5(a)(6).

Contents of forms
    Sec. 5. The registration forms prescribed under section 1 of thischapter must:
        (1) provide for the residence address and the mailing address of

the individual completing the forms;
        (2) contain a statement that a notice of disposition of theperson's registration application will be mailed to the mailingaddress of the individual; and
        (3) require the applicant to provide the applicant's voteridentification number.

Duties of circuit court clerk or board of registration
    Sec. 8. A circuit court clerk or board of registration shall providea sufficient number of affidavits of registration for the registration ofvoters residing in the county. The clerk or board shall also prepareand provide all other supplies needed to permit the registration ofvoters as provided in this title.
