    Chapter 41. Name Changes on Registration Records

Name change
    Sec. 1. A voter whose name is changed may vote at the electionby filing with the circuit court clerk or board of registration a verifiedstatement.

Time for filing verified statement
    Sec. 2. The statement described in section 1 of this chapter maybe filed with the county voter registration office at any time. A voterwho wishes to indicate that the voter's name has changed may alsowrite the necessary information concerning the name change on thepoll list under IC 3-11-8-25.1 before the person receives a ballot. Theperson may then vote if otherwise qualified.

Change made to voter's registration record
    Sec. 3. If a voter indicates a change of name on the poll list undersection 2 of this chapter, the county voter registration office shallchange the name of the voter on the registration record of theprecinct.

    Sec. 4. As required under 42 U.S.C. 1973gg-3(a)(2), the circuitcourt clerk or board of registration shall amend a voter's registrationrecord under this chapter to reflect information stated by the voter ona registration form submitted at a license branch.

    Sec. 5. The circuit court clerk or board of registration shall amenda voter's registration record under this chapter to reflect informationstated by the voter on a registration form submitted at a voterregistration office established under this article.
