Chapter 14. Replacement License

Replacement permit or license
Sec. 1. If a permit or license issued under this article, exceptsection 2 of this chapter, is lost or destroyed, and as provided insection 3.5 of this chapter, the individual to whom the permit orlicense was issued may obtain a replacement if the individual doesthe following:
(1) Furnishes proof satisfactory to the bureau that the permit orlicense was lost or destroyed.
(2) Pays the required fee for a replacement permit or licenseunder IC 9-29-9.

Restrictive and conditional licenses
Sec. 2. (a) This section applies to replacement licenses issuedunder the following:
(1) IC 9-30-4-6.
(2) IC 9-24-15.
(3) IC 9-30-5.
(4) IC 9-30-6.
(5) IC 9-30-10-9.
(6) IC 9-30-10-13.
(b) The bureau may issue a replacement license that is restrictiveor conditional if an applicant does the following:
(1) Applies for a valid replacement license.
(2) Surrenders all licenses in the applicant's possession.
(3) Pays the license fee under IC 9-29-9.

Name and address changes
Sec. 3. If a licensee or permittee who changes mailing address orresidence address or name, by marriage or otherwise, desires to havea replacement license or permit indicating the new address or nameof the licensee or permittee, the licensee or permittee may request theissuance of the replacement license or permit upon properapplication and the payment of the required fee as authorized by thisarticle.

Replacement license or permit by electronic service
Sec. 3.5. (a) The bureau may adopt rules under IC 4-22-2concerning the ability of an individual to apply for a replacement ofan operator's, a motorcycle operator's, a chauffeur's, or a publicpassenger chauffeur's license or a learner's permit to the holder of the
license or learner's permit by electronic service. If rules are adoptedunder this subsection, the rules must provide that issuance of areplacement license or learner's permit by electronic service issubject to the following conditions:
(1) A valid computerized image or digital photograph of theindividual must exist within the records of the bureau.
(2) The individual must be a citizen of the United States, asshown in the records of the bureau.
(b) An individual applying for a replacement of an operator's, amotorcycle operator's, a chauffeur's, or a public passenger chauffeur'slicense or a learner's permit must apply in person at a license branchif the individual is not entitled to apply by mail or by electronicservice under rules adopted under subsection (a).

Violations
Sec. 4. A person who violates this chapter commits a Class Cinfraction.
