34-13-4. Application for license--Contents. Any person, partnership, limited liability company, association, or corporation desiring a license under this chapter shall file with the board a verified application containing the name, age, residence, and citizenship of the applicant; the type of institution to be operated; the location thereof; the name of the person in charge thereof; and whether the minimum standards set by the board have been met and such other information as the board may require. Application on behalf of a corporation or association shall be made by any two officers thereof.
