340.050 Ā Form of petition. Ā A proceeding may be instituted under this chapter by the filing of a petition which shall be sufficient if it sets forth:

1. Ā The name of the petitioner.

2. Ā A description of the property, sufficient for the identification thereof, to which there may be attached a plat or map thereof.

3. Ā A statement that the acquisition of such property by the petitioner is necessary for a public works project and a brief general description of the public works project.

4. Ā A statement that the proceedings are being instituted under this chapter.

5. Ā A suitable prayer for relief.
