432A.150 Ā License: Contents. Ā Each license issued by the Bureau must contain:

1. Ā The name of the person or persons authorized to operate the licensed facility;

2. Ā The location of the licensed facility or, if the license is for an outdoor youth program, the area of operation of the program; and

3. Ā The number of beds authorized in the licensed facility, the nature of services offered and the service delivery capacity.
