449.085 Ā License: Form; contents. Ā Each license issued by the Health Division shall be in the form prescribed by the Division and shall contain:

1. Ā The name of the person or persons authorized to operate such licensed facility;

2. Ā The location of such licensed facility; and

3. Ā The number of beds authorized in such licensed facility, the nature of services offered and the service delivery capacity.
