450B.500  Do-not-resuscitate identification: Contents.  Each do-not-resuscitate identification issued by the health authority must include, without limitation:

1.  An identification number that is unique to the qualified patient to whom the identification is issued;

2.  The name and date of birth of the patient; and

3.  The name of the attending physician of the patient.
