451.160 Ā Notice to cemetery authority by friend or relative: Contents. Ā The notice to the cemetery authority shall specify:

1. Ā The name of the person whose remains are to be disinterred.

2. Ā As accurately as possible, the plot where the remains are interred.

3. Ā The date of interment.

4. Ā An address at which the required notices may be given by the cemetery authority.
