611.180  Invoice to be given to applicant; contents.  Every licensee shall give to every applicant for employment from whom a fee is required for securing employment an invoice on which is stated:

      1.  The name and address of the employment agency;

      2.  The name and address of the employer;

      3.  The name of the applicant;

      4.  The date;

      5.  The amount of the fee required for the services of the employment agency;

      6.  The title of the position secured by the applicant;

      7.  The name of the person authorized to hire employees for the employer; and

      8.  The amount of the gross cash wage.
