616C.005 Ā Forms for reporting injuries: Insurer to distribute revised forms to employers periodically; Administrator to make revised forms available to physicians and chiropractors. Ā On or before September 1 of each year:
1. Ā An insurer shall distribute to each employer that it insures any form for reporting injuries that has been revised within the previous 12 months.
2. Ā The Administrator shall make available to physicians and chiropractors any form for reporting injuries that has been revised within the previous 12 months.
