A. The registrant shall file a statement with the Commissioner including:
1. The name and address of the registrant and the name and address of theperson whose name will appear on the label, if other than the registrant;
2. The name of the pesticide;
3. A complete copy of the labeling accompanying the pesticide and a statementof all claims made and to be made for it including directions for use;
4. If requested, a full description of the tests made and the results thereofupon which the claims are based; and
5. Other information requested by the Board such as product efficacy, allknown health and environmental impacts, and known incidents of human orwildlife illnesses.
B. In the case of renewal of registration, a statement shall be required onlywith respect to information different from that furnished when the pesticidewas last registered or in response to additional requirements imposed by theBoard.
