§ 32.1-319. Written verification of application, statement or form; penaltyfor false or misleading information.

The State Board of Medical Assistance Services may require in its Plan forMedical Assistance that any application, statement, or form filled out bysuppliers of medical care under medical assistance shall contain or beverified by a written statement that it is made under the penalties ofperjury and such declaration shall be in lieu of any oath otherwise required,and each such paper shall in such event so state. Any person who knowinglyand willfully makes or subscribes any such papers or forms containing anyfalse or misleading information shall be guilty of a Class 4 misdemeanor.
