85.41 REFUSAL TO FURNISH STATEMENT.
On failure of the employer to furnish such statement of earnings
for thirty days after receiving written request therefor from an
injured employee, the employee's agent, attorney, dependent, or legal
representative, such employer shall be guilty of a simple
