135H.5  APPLICATION FOR LICENSE.
 An application for a license under this chapter shall be submitted
 on a form requesting information required by the department, which
 may include affirmative evidence of the applicant's ability to comply
 with the rules for standards adopted pursuant to this chapter.  An
 application for a license shall be accompanied by the required
 license fee which shall be credited to the general fund of the state.
 The initial and annual license fee is twenty-five dollars.
