144.39 CHANGE OF NAME.
Upon receipt of a certified copy of a court order from a court of
competent jurisdiction or certificate of the clerk of court pursuant
to chapter 674 changing the name of a person born in this state, the
state registrar shall amend the certificate of birth to reflect the
new name. A fee established by the department by rule based on
average administrative cost shall be collected to amend the
certificate of birth to reflect a new name. Fees collected under
this section shall be deposited in the general fund of the state.
