19:4-4.2.Ā Form and contents of statement
The statement shall be in writing, in such form as the Secretary of State may prescribe from time to time, and shall be sworn to under oath, or affirmed,Ā by the person named therein.Ā Every such statement shall set forth factsĀ showing the affiant's qualifications as a voter, a description of the placesĀ where he previously resided and the dates thereof, a description of the placesĀ where he resides, the names and all residences of the members of his immediateĀ family or household who are of voting age with indication of the residence atĀ which each is domiciled for voting, and a statement of the place of residenceĀ where he intends his domicile to be.
