SECTION 41-9.1-10

   § 41-9.1-10  Content of proposal. –The proposal filed with the state lottery division shall be made under oath,and shall include without limitation all of the following:

   (a) The name and business address of the applicant, and thenames and business addresses of the board of directors and the key officersthereof. In the event that a person or entity directly owns or controls a fivepercent (5%) or greater voting interest in the applicant, then the applicantshall also disclose the names and business addresses of such person or entityand the officers and directors thereof unless such entity is an institutionalinvestor in which case the name and address of the institutional investor needonly be disclosed. The applicant shall also disclose whether it has knowledgethat any disclosed person or entity has been convicted of any felony crime.

   (b) A fully executed development agreement between theapplicant and the Town of West Warwick.

   (c) A description of the proposed gaming operation andrelated amenities, including the economic benefits to the host community andthe state (i.e., the proposed amount of investment in construction anddevelopment; square footage of the casino; the number and types of games; thepresence of hotels, restaurants and other non-gaming amenities; parking spaces;etc.).

   (d) A description of the physical location of the proposedgaming operation and related amenities, and evidence that applicant has thereal estate site control (i.e., real estate purchase and sale agreements oroption agreements) necessary to support such development.

   (e) A description of the anticipated or actual number ofemployees, and related wages and benefits.

   (f) A description of the marketing and operating experienceof the applicant, and a description of how such experience and other assets ofthe applicant would enable it and the state to secure and maintain a strategicand competitive position within New England's casino gaming industry.

   (g) A statement regarding compliance with federal and stateaffirmative action guidelines.
