SECTION 42-66.4-3

   § 42-66.4-3  Patient assessment. – (a) The services defined and delineated in § 42-66.4-2 shall be providedonly to those patients who are medically eligible and who are in a skillednursing and/or intermediate care facility. Patients recently admitted shall beassessed as soon as possible. Provisions of long-term home health care servicespaid for by government funds shall be based upon a comprehensive assessmentthat shall include, but not be limited to, an evaluation of the medical, socialand environmental needs of each applicant for these services or programs. Theassessment shall also serve as the basis for the development and provision ofan appropriate plan of care for the applicant. The assessment shall becompleted by persons designated by the director, including, but not limited to,the applicant's physician and a representative of the department of elderlyaffairs.

   (b) Continued provision of long-term home health careservices paid for by government funds shall be based upon a comprehensiveassessment of the medical, social and environmental needs of the recipient ofthe services. This assessment shall be performed at least every one hundredeighty (180) days by the department of elderly affairs' case manager.
