SECTION 5-20.7-12

   § 5-20.7-12  Issuance of certificate orlicense – Certificate or license number. – (a) A certificate or license issued pursuant to the provisions of this chaptershall bear the signature of the chairperson of the board and a certificatenumber assigned by the director.

   (b) Prior to the issuance of any trainee license, the traineeshall provide to the department a notarized affidavit signed by the certifiedappraiser who shall directly supervise the trainee. The affidavit must providethe business name, address and license number of the certified appraiser.
