SECTION 5-37-26

§ 5-37-26 Medical forms. – Physicians or other persons are prohibited from requiring patients or theirrepresentatives to sign blank forms provided by or for any third party payersor nonprofit medical service corporations prior to, during, or subsequent torendering services. Any physician or other person violating any of theprovisions of this section shall be fined up to one hundred dollars ($100) foreach violation.
