§  9-128.  Canvass;  return  of.  1.  Returns  of the canvass shall be  printed in a form approved by the state board of elections. When  voting  machines  are  used the designating number and letter on the counter for  each candidate shall be printed next to  the  candidate's  name  on  the  statements of canvass.  The form of such return, for a general election,  also  shall  provide  for  the  entry  of  the  number of votes for each  candidate, or upon each ballot proposal, cast by absentee and  military,  special  federal, special presidential and emergency ballots, the number  of such votes cast by machine and the total number of such  votes,  cast  by such ballots and by machine together.    2.  One  set of returns shall be used, in each such election district.  Tally sheets shall be provided and used in each district  for  votes  on  paper,  absentee and military, special federal, special presidential and  emergency ballots cast during voting machine breakdowns, and  the  state  board  of  elections  shall  devise a form of tally sheet to be used for  that purpose, with provision for making the tallies as to  all  offices,  or all ballot proposals, on one sheet.
