<br><br> <br>Page 1 of 1 <br>14A.2-050 Forms. (Effective January 1, 2011) <br>(1) The Secretary of State may prescribe and furnish on request forms for: <br>(a) An application for a certificate of existence; <br>(b) An application for a certificate of authority; <br>(c) An amended application for a certificate of authority; <br>(d) A certificate of withdrawal; <br>(e) A change of registered office, registered agent, or both; <br>(f) A change of principal address; <br>(g) An application for a reserved name; <br>(h) The renewal of a reserved name; <br>(i) The transfer of a reserved name; <br>(j) The annual report; and <br>(k) An amendment to the annual report. <br>(2) If the Secretary of State so requires, the use of some or all forms listed in subsection <br>(1) shall be mandatory. <br>(3) The Secretary of State may prescribe and furnish on request forms for other <br>documents required or permitted to be filed by this chapter, but their use shall not <br>be mandatory. <br>Effective: January 1, 2011 <br>History: Created 2010 Ky.
